How do I schedule an appointment?

You can schedule an appointment by calling 443-628-8346 or through the request service link. Please know that it is faster and easier to request services through the link.

What new patient forms do I need to fill out?

Follow this link to complete forms before your appointment. Forms can be emailed to justducky@justduckybehavioralhealth.com, faxed to 443-557-1761, or brought in during your first appointment. You will also receive a link to the required forms and assessments by signing up for the portal. 

You cannot be seen if we do not have your paperwork, a copy of your ID and insurance card by the your appointment.

What should I expect when I arrive?

Please plan to arrive 15-20 minutes prior to your scheduled appointment time if you have yet to complete new patient form(s). You will be greeted by our receptionist as you “check-in” at the front desk.  She will ask to see your insurance card and a photo ID.  Shortly thereafter you will be called in with the counselor. If you are doing a telehealth appointment you will need to pay your co-pay at check-in.

Will I need to be prepared to make a payment at this time?

You will be responsible for any co-pays reflected on your insurance plan card.  If you have specific questions concerning your benefits, please call the member services phone number on your insurance card. 

What should I expect after my appointment is completed?

Our receptionist will schedule follow-up appointments as directed by your counselor if you have an in person session. If you see your counselor by tele-health the counselor will schedule with you.
